A Crown Heights, NY Medicaid and Homecare company is hiring a service coordinator. Job includes managing a case load of patients by assisting them with finding medical providers, coordinating patient transportation, getting medical supplies, and accessing community resources. Job involves maintaining contact with the families and completing all required documentation. Once every 3 months the candidate will be required to do a house visit (transportation is provided). Candidate must have a bachelor's degree or an associate's degree. Looking for someone who is compassionate with excellent communication skills and at least 6 months of office experience in any field. Benefits include PTO and holiday/vacation pay. Salary is commensurate with experience and hours.
Mon-Thurs, 9 am-5 pm (must work until 5 pm), and Fri, 9 am-1/2 pm
