Career Services Case Manager (Workforce)

About the role

JOB SUMMARY We are seeking a Career Services Case Manager (Workforce)  to join its Career Services Department.  The Case Manager will provide individualized support to program participants as they work toward employment, education, training, and long-term career goals.

ABOUT YOU You are a motivated, compassionate and highly organized Case Manager. You have  experience in case management, workforce development, career coaching, or human services and understand how to build trusting relationships with individuals who may face barriers to employment. This position requires strong communication, documentation, problem-solving, and follow-through skills.

WHAT YOU'LL DO • Conduct participant intake, eligibility screening, and comprehensive needs assessments.

• Develop individualized service plans based on each participant's employment, education, and career goals.

• Provide ongoing case management, career coaching, and motivational support.

• Assist participants with résumé development, job applications, interview preparation, workplace readiness, and job-search strategies.

• Connect participants to employment opportunities, occupational training, educational programs, benefits, and supportive services.

• Identify barriers to employment and coordinate appropriate referrals to internal and community-based resources.

• Maintain consistent communication with participants and conduct timely follow-up.

• Monitor participant progress toward established goals and required program milestones.

• Maintain accurate, confidential, and timely case notes, participant files, attendance records, referrals, and outcome documentation.

• Enter and update participant information in required databases and tracking systems.

• Support participant recruitment, outreach, orientation, workshops, and program events.

• Collaborate with Career Services staff, employers, training providers, educational institutions, and community partners.

• Assist with employment placement, retention tracking, and follow-up services.

• Ensure that services and documentation comply with grant requirements, agency policies, and established deadlines.

• Participate in team meetings, case conferences, professional development, and program evaluation activities.

• Perform other related duties as assigned.

OUR REQUIREMENTS • Bachelor's degree in social work, human services, psychology, counseling, education, workforce development, or a related field.

• At least two years of relevant experience in case management, workforce development, career coaching, employment services, or human services.

• Demonstrated ability to engage and build trust with individuals from diverse backgrounds.
