Position OverviewWe are seeking a compassionate and detail-oriented Patient Liaison to oversee a caseload of approximately 25 patients participating in the NHTD program. The individuals served are living with dementia or cognitive decline and require consistent coordination, advocacy, and support. This role offers meaningful, hands-on experience within a collaborative and supportive healthcare environment.ResponsibilitiesConduct regular phone check-ins with families and caregiversDe-escalate and resolve concerns with service providersSchedule therapies and transportationComplete renewal paperwork and incident reportsConduct in-home visits once per quarter throughout the five boroughsTraining & Schedule4–6 weeks of paid, in-person training in BrooklynHybrid schedule after training (remote work + quarterly home visits throughout the 5 Boroughs)QualificationsAssociate’s or Bachelor’s degree requiredStrong communication and organizational skillsRelated healthcare or human services experience is a plus, but not required
